Employee Corrective Action Form

Statement of Purpose

The Company has guidelines and standards in administering corrective action to address poor job performance and/or misconduct in the workplace. The purpose is to:

  • Address issues in a fair and consistent manner
  • Mitigate risk to the Company and the manager

The Corrective Action process applies to employees with poor performance and/or behavior. Depending on how serious or repetitive the issues are, the employee may move to one of two more targeted paths that are part of the Corrective Action process (or both paths if necessary). These two paths are Progressive Discipline and the Performance Improvement Plan (PIP).

When to Use this Form

The Employee Corrective Action Form should be used to document the first two steps of Corrective Action (Counseling and First Written Warning) as well as the subsequent steps of the Progressive Discipline path.

For subsequent steps of the Performance Improvement Plan process, please use the Performance Improvement Plan form.
